PROVIDER PAU HANA
The Provider Pau Hana on Monday, July 22, 2024 at the Hilo Yacht Club, brought together providers from our East Hawai’i Community to build lasting relationships and collaborate on improving healthcare on Hawai’i Island.
Communication and cooperation for better outcomes.
We believe that long-term healthcare solutions for Hawaiʻi Island communities require cooperation, whether that’s at the patient-provider level or the systems level. Community First has established and coordinates two islandwide networks of individuals and organizations working in the areas of healthcare and social services on Hawaiʻi Island: the Community Action Network and the Regional Health Partnership.
Community Action Network
The Community Action Network (CAN) brings together frontline representatives from over 40 social service, medical, and healthcare organizations to address the social determinants of health for patients with significant healthcare needs. Members attend monthly meetings to share information about available services and develop strategies for improving patient care while curbing avoidable hospital utilization.
Members of the Community Action Network include case managers, community health workers, care coordinators, paramedics, and service provider representatives. The Community Action Network has been highly useful for sharing resources, making referrals, and conducting palliative care outreach discussions.
Regional Health Partnership
The Regional Health Partnership (RHP) is a network of executive-level representatives from the County of Hawaiʻi, health plans, medical groups, and social services who focus on systems-level initiatives to improve our healthcare system. Since 2015, the Regional Health Partnership has led efforts to improve coordination between providers and establish common ground to support long-term partnerships.
The network brings together decision-makers from healthcare provider organizations, businesses, government, health plans, and community organizations on a bi-monthly basis.
