Connecting Hawaiʻi Island Healthcare and Social Service Workers

Healthcare and social service silos are all too common on Hawaiʻi Island, leading to avoidable hospitalizations and financial hardships for our most vulnerable community members. This is compounded for folks who also have complex non-medical needs and face social, economic, or geographic barriers to accessing healthcare. 

The Community Action Network exists to connect frontline healthcare and social service professionals to collectively address the social determinants of health in our community, including transportation, food insecurity, and housing. We meet once a month and provide a safe, open forum for discussion. We also have an online inventory to facilitate referrals among network organizations. Members of the Community Action Network include case managers, community health workers, care coordinators, paramedics, and service provider representatives.

The Community Action Network is currently led by Nic Los Banos of the Alzheimer’s Association and Karen Teshima of Blue Zones Project/Sharecare.
