Regional Health Partnership (RHP)

As its main priority the RHP is currently focused on improving care for the high cost, high need patient (HCHN) who have many non-medical needs. These social determinants of health which impact health outcomes and therefore medical utilization and costs are the focus of social service providers, and thus the Community Action Network (CAN) was created to coordinate medical and social services. Through CAN consensus was reached regarding a care model for HCHN patients and developing centers of excellence in East Hawaiʻi to address complex needs of every HCHN patient and establish continuum of care with their medical team/home. An infrastructure to support this care model is being developed and includes an online inventory of resources and referral management, a Community QA Committee, and Exception Funding. Our work also proceeds from the assumption that in East Hawaiʻi only together as a community can we make significant progress.
