Healthcare for Serious Illness

The primary goal of the PTPC project is to develop a system that connects individuals living with serious illnesses to quality palliative care, beginning with a pilot project in East Hawaiʻi. While there are many challenges to delivering palliative care, PTPC efforts will focus on systemic changes within health and social service sectors.

The intent of this project is to establish a model that guides the process of identification, education, and ultimately a connection with palliative care that fits patient and family needs. In general, this project will support the development of an efficient process built upon three foundational elements: community partnerships, community and provider education. These elements align with Stupski Foundation’s palliative care strategies: awareness, education, and acceptance, community-based palliative care, and supportive healthcare systems.

VISION: Families have access to quality palliative care.

MISSION: Enhance awareness and access to quality palliative care by developing a collaborative, community-based system identification, education, and support for individuals with serious illness and their families.
